What You Need to Know for Your Notary Appointment

ACCEPTABLE IDENTIFICATION:

 Any one of the following forms of identification (ID) is sufficient, provided that it is current or was issued within 5 years: 

  • An ID card or driver’s license issued by the California Department of Motor Vehicles; 
  • A United States passport; 
  • An inmate ID card issued by the California Department of Corrections and Rehabilitation, if the inmate is in custody in California state prison; 
  • Any form of inmate ID issued by a sheriff’s department, if the inmate is in custody in a local detention facility; 

 

Any one of the following forms of ID, provided that it also contains a photograph, description of the person, signature of the person, and an identifying number: 

  • A valid consular ID document issued by a consulate from the applicant’s country of citizenship, or a valid passport from the applicant’s country of citizenship; 
  • A driver’s license issued by another state or by a Canadian or Mexican public agency authorized to issue driver’s licenses; 
  • An ID card issued by another state; 
  • A United States military ID card; 
  • An employee ID card issued by an agency or office of the State of California, or an agency or office of a city, county, or city and county in California; 
  • An ID card issued by a federally recognized tribal government. 


Please be advised that we DO NOT notarize blank documents under any circumstances. All documents that you present must have all the pages that belong to the documents (for example, all 6 of 6 pages, not just the page that needs notarizing), and all applicable blanks spaces or lines filled in either with relevant information or with the words "Not applicable" or "N/A" (to prevent fraud or other problems that may surface later). Note: for jurats or affidavits, you must leave the signature line blank because the Notary has to actually witness the document being signed.

Notaries in California are authorized to notarize documents by performing an Acknowledgment, Jurat, or Oath. You can download the California All-Purpose Acknowledgment Form, or the California Jurat Form.  There is not a prescribed general form for an Oath.

Primary ID - Fingerprint

What You Need to Know for Your Live Scan Appointment

Please bring your Live Scan form from the requesting agency (with your information completed) and an acceptable ID.


ACCEPTABLE IDENTIFICATION: 

     Non-expired Approved Primary Identification. 

     Approved ID (Bring 1 of the following):

  • State-issued driver’s license
  • U.S. Passport or U.S. Passport Card
  • Federal Government Personal Identity Verification Card (PIV)
  • Uniformed Services Identification Card
  • Department of Defense Common Access Card
  • Foreign Passport with Appropriate Immigration Document(s)
  • USCIS – Permanent Resident Card (I-551)
  • USCIS – Employment Authorization Card (I-766)
  • Federal, state, or local government agency ID card with photograph
  • U.S. Coast Guard Merchant Mariner Card
  • Canadian driver’s license
  • State ID card if the state’s identification card standards are the same as for the driver’s license

Secondary ID - Fingerprint

Secondary Forms of Identification 

In the absence of a primary form of identification, an applicant shall provide at least two of the following forms of secondary identification AND at least two additional forms of the supporting documentation that follows: 


ACCEPTABLE SECONDARY IDENTIFICATION 

      Approved ID (Bring 2 of the following):

  • State Government Issued Certificate of Birth
  • U.S. Tribal or Bureau of Indian Affairs Identification Card
  • Native American tribal document
  • Social Security Card
  • Court Order for Name Change/Gender Change/Adoption/ Divorce
  • Marriage Certificate (Government Certificate Issued)
  • U.S. Government Issued Consular Report of Birth Abroad
  • Draft record
  • School ID with photograph (minimum age 7 years old, or transaction will be rejected by DOJ) *
  • Certificate of Citizenship (N560)
  • Replacement Certificate of Citizenship (N-561)
  • Certificate of Naturalization (N550)
  • Replacement Certificate of Naturalization (N-570)


PLUS Two Supporting Documentation for Secondary Forms of Identification (In addition to the two above, bring 2 of the following):

  • Utility Bill (Address)
  • Jurisdictional Voter Registration Card
  • Vehicle Registration Card/Title
  • Paycheck Stub with Name/Address*
  • Jurisdictional Public Assistance Card
  • Spouse/Parent Affidavit
  • *Financial information may be redacted (hidden) by the individual.
